Steps to enroll:

Complete the new student online registration form for your new student(s) by using the link above.
You must schedule an enrollment appointment by calling (419) 782-0070 after submitting registration starting June 2025. Your enrollment appointment will be at the Defiance City Schools Administrative Offices located at 801 South Clinton Street Defiance, Ohio 43512. Your child’s enrollment is not considered complete until after the enrollment appointment and all supporting documentation, listed below, has been submitted.
Please bring the following documents to your appointment:

Birth certificate
Immunization records
Proof of residency, such as a utility bill, lease agreement, insurance bill or bank statement that lists your name, address and a date within the last 30 days.
Custody papers if applicable
IEP and ETR if applicable

Note: If you do not have access to a computer please contact the central office at 419-782-0070 to schedule an appointment at the Defiance City Schools Administrative Offices where a computer is available for you to enroll your child online.

2. Open Enrollment

If you are interested in Open Enrollment to Defiance City School, apply below:

The District Office will receive this form immediately after you complete it. After completion you will be notified via phone call whether it was approved or denied. If approved, you will click on link above to register your new student.

3. Emergency Medical and Transportation Forms

This section is for updating student's information that are currently enrolled in Defiance City Schools.

Every school year, parents MUST update each child's Emergency Medical and Transportation Information prior to the start of the school year. In order to complete the electronic Emergency Medical and Transportation form you must log into your PowerSchool Parent Portal: PowerSchool

Please be aware, you can not use the PowerSchool app to make these changes.

Parents: This is required every school year for all of your children attending Defiance City Schools. It is IMPERATIVE to complete this online process in order to update addresses, phone numbers, health concerns, emergency contact information and transportation requests, etc. Students are not permitted to attend school events such as school dances or field trips until the Emergency Medical and Transportation Form has been completed.

Select the Emergency Medical and Transportation Form Icon on the bottom left side

What if I can’t remember my login for the PowerSchool Parent Portal?

If you aren’t able to retrieve your login credentials using the “Forgot Username or Password” link on the Sign In page, please contact your school directly for assistance logging into the Parent Portal.

Defiance City Schools offers online registration for new students entering the district. This includes students that have attended Defiance City Schools before, withdrew and are returning back into the district.

Parents must complete the new student online registration to enroll or re-enroll their child. The online registration must be completed prior to scheduling an appointment with the central office to complete paperwork, which finishes the new student enrollment process.
